Hospitalization Procedures

The doctor or dentist will decide if hospitalization is required according to the results of the examination and diagnosis. 
As needed, the Admissions/Discharge Center will explain hospitalization preparations and collect information regarding emergency contacts, living arrangements, etc. (Obstetrics, Psychiatry and Neurology will carry it out by each department.)

Once the date of your hospitalization is decided, the doctor in charge or the Admissions/Discharge Center will notify you by telephone or postal mail. 
If you are unable to keep the designated date or wish to cancel your hospitalization, please call 0166-65-2111 (rep.) and notify the department in charge of your hospitalization.

On the day of the hospitalization, please come to the hospital at the specified time, and follow the procedures for hospitalization at the Admissions/Discharge reception counter. 
Please submit the following documents during the hospitalization procedures.

  1. Health insurance card or Latter-Stage Elderly Healthcare System insurance card
  2. Beneficiary certificate (disability, infant, single parent, etc.)
  3. Hospitalization application/Guarantee form (provided at the time of hospitalization application)* Please be sure to fill in the guarantor field. 
  4. Application form to rent hospital clothes (provided at the time of hospitalization application) 
  5. Confirmation form of hospitalization history (provided at the time of hospitalization application)* 
  6. Patient ID card (used at the time of outpatient examinations) 
  7. Certificate of eligibility for ceiling-amount application, Certificate of reduction of the standard amount of patient liability (eligible patients)

※Of the above items, your Health insurance card or Latter-Stage Elderly Healthcare System insurance card, Beneficiary certificate, Certificate of eligibility for ceiling-amount application, and Certificate of reduction of the standard amount of patient liability will be promptly returned.

Your patient ID card will be returned at the time of discharge.

If you are hospitalized for injury due to traffic accident or while working, please be sure to notify the Admissions/Discharge reception counter.
